Advanced Filter between 2 dates and more then one occurence

Hi,

Trying to work through an advanced Filter.
How do I complete the filter between 2 dates and also filter if the name is there twice. I have an appointment booking database and want to create an alert if the person has been booked X amount of times in a week(lets say 3)

Headings are:
Date of Appointment & Name

Not sure how to complete both of these functions in one Advanced Filter

Thanks for looking over this post.
I hope this generates more feedback.

I added a basic view of what I am working with,
So I would like for the filter to only show 1-the bookings between the two dates and 2- if there are any people with more then 2 bookings during that time period.


The third part would be also get some help to create an alert box if some sort to notify the user.

I should also add that the two criteria dates are being pulled from another sheet
Thus ...
Date 1 =B1 on worksheet 1
Date 2=B10 on worksheet1
