Add Column Query

Hi All, I have successfully got the above formula working but I need to have this step within my query as the data it presents requires a remove duplicates step after-I am sure the custom add column could do this just not sure how to go about this

Would be greatful for any assistance

Need 1 part of a formula to always point to the same range? use $ signs: $V$2:$Z$99 will always point to V2:Z99, even after copying
Are you saying that before this step there are no duplicates but after this step there are duplicates?
There are duplicates prior and after-the column that this will create will have duplicates for the same operator which is fine as I can now remove one of the rows as the present of both indicates a certain status that I am trying to count/track. See below
