ActiveX checkbox copy to another sheet.

Hello. I am new to vba scripting and any help would be greatly appreciated.

I have a spreadsheet that has multiple sheets. The "User Access Form" sheet has several activex checkboxes. I would like to copy the fields that are checked to the "Info" sheet, which is currently blank.
