A return that is resulting from matching multiple conditions

Aloha

In Access, I have a table with three parts
  1. Specialty
  2. Location
  3. List of support names
In my form I want to have a field to enter number 1 and number 2 the result in a different cell will return the correct name. B The attached images is from my table.

The first two are the condition that needs to match to return columns 2,4,5 etc. Each return will be a different cell in a form or report. Example: A chief could be more than one location but with the same specialty or vise versa.

I am stumped on what I need to do.
