a formula to display several strings in a structured way with conditions

Hello All,

I am looking for a formula or a trick to display several values (strings: phrases) in the same cell and in an organized way (I mean return to line) with 2 condtions ..

If you could help me guys, I will appreciate it even after the incarnation of my soul :)

Excel Facts

Is there a shortcut key for strikethrough?
Ctrl+S is used for Save. Ctrl+5 is used for Strikethrough. Why Ctrl+5? When you use hashmarks to count |||| is 4, strike through to mean 5.

here is a solution (sort of) with helper columns F to I.
it setup for max 4 procedureNames per procedureType that can be extended.
