vlookup - matches not made

Vlookup is not displaying all appropriate matches - only some of them. I have tried adjusting the cell formatting to be consistent using "general." I've never had this problem before.
Any ideas?

By chance, have you...
...omitted the 4th (optional) argument to the VLOOKUP
function? Or, have you used 1 or TRUE as the 4th
argument?
