HYPERLINK Function Error

Hi, I am trying to create a dynamic link, which will link to the same work book but different sheet, I am using Excel 2010.

My current formula is as follows:
=HYPERLINK("#'Current Portfolio Tracker 2015'!B7"&MATCH(Index!A5,'Current Portfolio Tracker 2015'!$B:$B,0))

My original hyperlink tab is in Current Portfolio Tracker Cell B7.
I want to reference it on my index page Cell G5 so that when I click on it on the Index, Cell G5, it will take me to the tab.
