Lookup with multiple values

I'm trying to find a formula that I know already exists, but I can't find it for the life of me. It's a formula that nests two index formulas. I need to lookup two different values in two columns to find the unique row where the data resides that I want to pull.

It was something like:

=index(range, match(cell1&cell2(index(range&range,,,), column#)

Any help appreciated!
 
=INDEX($F$330:$F$333,MATCH(D322&E322,INDEX($D$330:$D$333&$E$330:$E$333,0),0),1)

Obviously adjust your ranges to suit
